This is the 'Categories' form. To add a page with this form, enter the page name below; if a page with that name already exists, you will be sent to a form to edit that page.
This form allows the picking of categories for pages and it uses the CategoryTree and SemanticForms extensions. The template that generates the output to place pages in categories is Template:Categories.
